1. The Affiliation fee will be Rs. 20,000/- . You can select any two courses from the list of courses. For each additional course, Rs. 10000/- has to be paid.
2. An inspection fee Rs. 2000/- (Rupees two thousand only) also to be paid.
3. You have to remit the above mentioned fees through RTGS facility to National Skill India Development Council (NSIDC), A/C No. 29860100005144, Bank: Bank of Baroda, Branch: Peroorkkada, Thiruvananthapuram, IFSC: BARB0(zero)PEROOR.
4. You have to submit the following documents along with the fee remittance Chelan/DD:
- a. A request in your letter pad (softcopy by Email and hardcopy by courier/post) seeking affiliation of NSIDC for conducting desired courses.
- b. Application in the prescribed form (Application form can be downloaded from here)
- c. Stamp paper for Rs. 200/- taken in the name of Authorized Person of the centre to be affiliated.
- d. Self-attested copy of certificates of the faculties.
- e. List of instruments/equipment in lab/used for the training.
- f. A tie-up letter with any hospital/lab for giving practical training to the students (This is applicable only for Paramedical & Ayurveda courses).
- g. List of Computers and peripherals in the computer lab or a tie-up letter with a computer center to provide computer practical (for selected courses)
- h. Attested copy of any Photo ID of the Authorized Person to be signed on the MoU
- i. Attested copy of any document to prove the House address of the Authorized Person to be signed on the MOU (Copy of Passport, Telephone Bill, Bank Passbook etc).
- j. Passport size photo of the Authorized Person to be signed on the MoU
- k. Copy of local body (Panchayat/Municipality/corporation) License.
- l. Copy of Rent Agreement.
- m. Sketch of the institution arrangements (classroom/training hall, lab, office, toilet etc.).
- n. Photograph of the institution (one for outside view, one each for showing office, classrooms/training hall, lab, toilet etc.)
- o. List of library books.
- p. Copy of partnership deed (in case of partnership firm) OR articles and memorandum of association (in case of company) OR trust/society deed (in case of charitable society/trust).
- q. Copy of resolution that passed by the competent body to authorize a person to sign the MoU with NSIDC (It is applicable only for partnership firm, companies, trust and societies).
5. Send the above mentioned documents to the address: CHIEF EXECUTIVE OFFICER (CEO), NSIDC, BHARAT SEVAK SAMAJ, 3RD FLOOR, KORAMBAYIL ARCADE, PANDIKKAD ROAD, MANJERI-676121, KERALA.
